Alcoholism is a disease that includes the following four symptoms:
Craving—A strong need,
or urge, to drink.
Loss of control—Not being able to stop drinking once drinking has begun.
Physical dependence—
Withdrawal symptoms,
such as nausea, sweating, shakiness, and anxiety after stopping drinking.
Tolerance—The need to drink greater amounts of alcohol to get drunk.

Lovelace Case Management is a multi-disciplinary team consisting
of registered nurses, social workers, and Case Coordinators.
Case management assists members and families with complex care needs through a comprehensive
and objective analysis of members’ clinical, financial, and psychosocial needs. Case Management is provided
by telephone, and is available to all members covered under Lovelace Senior Plan. Care coordination services are available to assist members in accessing and coordinating services with physician offices, as well as home care and durable medical equipment providers. Case Management is a collaborative process. Lovelace Case Managers and Case Coordinators establish partnerships with the members, member’s families, physicians, facility case managers, and disease management staff, as well as with community-based resources as needed.

Referrals for Case Management services come from multiple sources. However, Case
Management will take place
only with the consent of the member or the member’s
power of attorney. Common sources of referral are:
• Members may self refer
• Member’s family
• Physicians
• Hospital case managers
• Community case managers
